Implement vhdl modules for single 40G interface - v1.6 firmware

Deployment on real TPM hardware was proposed for inclusion and completion of this feature. However even though this feature's work is functionally complete, unavailability of TPM hardware meant that this final activity did not occur, and instead will take place during PI12.
